Registrar's Office

The Registrar’s Office takes care of keeping and updating the records of the students.   It accomplishes college application forms and requisitions for transcript of records and other certificates. It accepts new applicants in the Grade School and High School Departments. It schedules the interview  of the applicants in coordination with the Student Development Center. It submits the enrolment data, promotion sheets and other requirements to the Department of Education. The office also takes care of the application for financial grantees and likewise the application for the educational benefits of the children of employees.

TRANSACTION GUIDELINES

With the nationwide implementation of the K to 12 Basic Education Program and strict compliance with the implementing rules and regulations of the Data Privacy Act of 2012, the following policies and reminders from the Office of the Registrar are hereby presented in light of education laws, guidelines and standards of Department of Education, Bureau of Immigration, and other agencies.

A. Requests for School Records
In general, confidential records may be released only to the owner of the record (if she is of legal age). Other than this, confidential records will be released under the following circumstances.
1. Upon request of a parent or legal guardian (in the case of currently enrolled students who are still minor or below 18 years old)
2. Upon receipt of a written request from the owner (in the case of graduate students) carried by any member of her family (See Authorization.)
3. Upon written consent given by the owner for academic information (See Records Verification.) 

B. Authorization
The authorization should contain:
1. The identity and personal circumstances of the owner e.g. student’s name, date of birth, period of stay in school, course graduated from and date of graduation.
2. The name of the representative and the relationship to the owner of the record.
3. The document being requested, the number of copies and the purpose (specific use) of the documents sought.
Sufficient proof of identity must be established both for the owner and her representative. This includes copies of, at least, two valid identification cards (driver’s license, passport, company ID, etc.) from both the owner and her representative. 

C. Records Verification
1. SSAM may verify the contents of copies of records sent to them in connection with the application for employment of the graduates provided that the requesting company/party attached a copy of the owner’s Letter of Consent.
2. No background investigation will be entertained over the phone.

D. Correction of School Records (For currently enrolled students only)
Change of name based on court decisions or Civil Registrar General’s action on petitions will be processed provided that the change is already annotated in the Certificate of Live Birth of the student issued by the National Statistics Office/Philippine Statistics Authority.

The correction of entry(ies) of currently enrolled students shall be made by the School Registrar upon submission of an original copy of Certificate of Live Birth issued by the PSA, and Affidavit of the parents/guardians of the student explaining the circumstances that led to the recording of the incorrect entry(ies).
